Texas (LPC-S Your Consumer Rights)
In accordance with Texas House Bill 4224 and the Texas Behavioral Health Executive Council (BHEC) rules:
Access to Records: You have the right to access your health records. To begin this process, please request a “Release of Information” form from me in writing.
Licensing Board Complaints: You have the right to file a complaint. Information and processes regarding complaints to the Texas Behavioral Health Executive Council can be found and completed online here: BHEC Complaints & Enforcement
Consumer Protection: You also have the right to file a consumer protection complaint with the Texas Attorney General regarding your protected health information here: Texas Attorney General – Consumer Protection
Supervision Services: I am a Board-Approved Supervisor. Texas LPC-Associates under my supervision must include my name and contact information on all advertisements, billings, and announcements.

Colorado (LPC)
Mandatory Disclosure for Colorado Clients The practice of licensed persons in the field of psychotherapy is regulated by the Mental Health Licensing Section of the Division of Professions and Occupations.
Sexual Intimacy: In a professional relationship, sexual intimacy is NEVER appropriate and should be reported to the Colorado Department of Regulatory Agencies (DORA).
Client Rights: You are entitled to receive information about my methods of therapy, techniques used, and fee structure.

Florida (Out-of-State Telehealth Provider)
Florida Telehealth Notice I am registered as an Out-of-State Telehealth Provider in Florida (Registration TPMC6767). I do not provide in-person services in Florida. In accordance with Florida Statutes § 456.47(4), you may verify my registration and find further information at the link below:

Louisiana (Registered Telehealth Provider)
Louisiana Teletherapy Notice I am an Out-of-State Telehealth Provider registered with the Louisiana LPC Board of Examiners. Complaints: Clients have the right to file a complaint in their home jurisdiction. Instructions on how to file a complaint can be found on the Louisiana LPC Board Website.
Vermont (Registered Telehealth Provider)
Vermont Telehealth Disclosure
I am registered with the Vermont Office of Professional Regulation (OPR) as an Out-of-State Telehealth Provider and am authorized to provide counseling services to clients located in Vermont through telehealth. I do not maintain a physical office in Vermont and do not provide in-person counseling services within the state. Telehealth services are provided in accordance with Vermont laws and regulations governing interstate telehealth practice.
Client Rights: Vermont clients have the right to confidentiality, informed consent, access to records, and professional services that meet Vermont's applicable standards of care. Records created through telehealth services are maintained in accordance with federal and Vermont privacy laws.
Complaints: Clients who wish to file a complaint regarding professional conduct may contact the Vermont Office of Professional Regulation.
